1. Submit a Referral

Case managers, social workers, healthcare providers, agencies, or prospective participants may contact us by phone, email, or through our online referral form.

2. Initial Screening

We conduct a brief screening to determine whether Anchor Haven Living is an appropriate fit based on our admission criteria and housing availability.

3. Phone Assessment

Each prospective resident completes an assessment to discuss their housing needs, goals, expectations, and overall readiness for our program.

4. Admission Decision

Once approved, we coordinate move-in details with the participant and, when applicable, their referring agency or case manager.

Move-in is just the beginning. We work alongside participants, case managers, and community partners to promote accountability, encourage personal growth, and support each person's journey toward lasting housing stability and independence.
